This notice covers the public MediMate website and pilot-access enquiries. Medical information entered in the app is covered by the app’s privacy notice and separate consent choices. Please do not send health reports, medication details or another person’s health information through this website form.
Who handles your information?

What the request form collects
The form asks for your email address, whether you are a patient or family member, carer, facility, professional, pharmacy or another interested person, and your agreement to use those details to review and respond to the request. It also sends a label identifying the submission as a pilot-access request. Providing these details is voluntary; without them we cannot respond through this form.
Why we use it
We use your request to assess interest in the pilot, respond to you and arrange appropriate next steps. Submitting it does not create an account, approve access or subscribe you to a newsletter. Please contact us if you want to withdraw a request or stop further contact about it.
Formspree and processing outside South Africa
The form sends your submission to Formspree, our form-processing provider, which can store submissions and deliver them to our email account. Formspree also receives technical information such as IP address, browser information, access time and the referring page. Its service uses infrastructure in the United States and may process information in other countries. Read Formspree’s privacy policy before choosing to submit. Website delivery and cookies
Website hosting and email services process the technical information needed to deliver pages, protect services and handle communications. This version of the public website does not add advertising or analytics scripts, and loads its page fonts from your device. The linked form provider and other external websites have their own privacy practices.
Keeping and deleting requests
Requests may be held in the form service and our email records while we review and respond to them and arrange any continuing pilot onboarding. Ask us for access, correction, deletion or retention details using the contact above. We will consider the request and explain any applicable legal or administrative reason for retaining information. Deletion from one system does not mean copies in backups or service-provider records are removed immediately.
Your choices and complaints
You may ask what personal information we hold about you, request access or correction, ask for deletion where applicable, object to processing or withdraw consent. We may need to verify your identity before providing information. You can also contact South Africa’s Information Regulator if you believe your information has been handled improperly.
Information in the app
Joining the pilot involves a separate invitation and onboarding process. Review the app’s privacy notice and consent choices before entering health information. Optional report explanations and consultation AI processing have separate consent choices and may involve processing outside South Africa. Website enquiry consent does not authorise AI processing of medical information.
